Please call us at least 24 hours in advance if you cannot keep your appointment so we may give your time to another patient in need of our care.
We participate with and accept most types of health insurances. Our staff will gladly tell you if we participate in your particular health plan. We gladly accept cash, check and credit cards, our policy is that you pay any amount not covered by your insurance at the time of your visit.
When calling our office, our receptionist will bring your questions to the attention of your physician or the nurse as soon as possible. Please give us until the end of the business day to return your calls.
When calling to refill your prescription, please call us at least 24 hours before you are out of your medication. Please have the name and dose of your medication as well as the name and phone number of your pharmacy available for our staff. Due to the control substance requirements you may be asked to make an appointment to get these medications filled.
Forms and copy fees
There is a charge of $30.00 for all disability papers. In order to reduce the cost of copying medical records we encourage you to log in your patient portal and print any records yourself, otherwise fees may apply if our office staff is requested to do that.
In case of emergency, call 911 or go directly to the emergency room of the nearest hospital and have the attendant call the neurologist on call. If you have the need for an urgent evaluation with our doctor, please call 989-799-2770 and inform our receptionist of your concern. She will make every effort to schedule an immediate appointment with the doctor.
